All Child Welfare Services – New System (CWS-NS) Project staff and contractors are required to complete the Conflict of Interest and Confidentiality Certification form before beginning work on the CWS-NS Project. The purpose of this form is to protect the State’s confidential information and to help prevent conflicts of interest on the part of CWS-NS Project staff and contractors. A copy of the form is located here. Once you have read and signed the form, please send your original completed form to:
